Your initial appointment typically will consist of an examination, updating any necessary X-rays, and review of your treatment options (please note a cleaning is not usually included in your first visit with us). When you have your first appointment scheduled, you may fill out our online forms to expedite your visit.

Please assist us by providing the following information at the time of your consultation:

• Any current X-rays taken by a previous dentist, if applicable. Bite-wing (cavity-check) X-rays are usually current for 12 months, and a pano or full-mouth series is usually current for five years.
• A list of medications list of medications you are presently taking.
• If you have dental insurance, bring all the pertinent information including your ID card if issued. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints,etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
